I can see on your page that some scriptys are loaded twice (sweetalert)
Can you disable the userpoints2paypal module so we can test.

- If you get a white page , enable error reporting and / or check your server error logs.

I disabled the module and it still isn't working. I will try and find you on Facebook to send you a developer request.
